Many states use “Application for Certificate of Authority,” but Arizona, Connecticut, Indiana, Iowa, Pennsylvania, Washington, and Wisconsin say “Foreign Registration Statement”; Maine says “Statement of Foreign Qualification”; New York says “Application for Authority”; and California and Texas say “Application for Registration.” Same filing, different label.
Texas and South Dakota charge $750 and Massachusetts $500, while Michigan is just $50 and Utah $59. Tennessee uses a per-member fee ($50/member, $300 minimum up to $3,000). Nevada bundles a mandatory $150 state business license into the cost.
